I have a young athlete who was scheduled for a CT scan w/contrast but his creatinine level was 1.4 and the test was postponed. He takes 5 gr of creatine each day and does heavy weight lifting for his college team. Any ideas? 

What is the effect of creatine supplementation on creatinine levels as measured in blood tests?

Creatinine and creatine/muscle creatine are not the same thing. Remember that creatine is found in meat products, or more specifically, in muscle that we eat. Most men on a normal diet get a bunch of creatine in their diet. However, high levels of creatinine can be associated with kidney problems and if an athlete has kidney problems they probably shouldn't be using creatine, but it depends on the problems. THAT BEING SAID, the high creatinine level of 1.4 is only slightly higher than the normal range of 1.27. Most likely the high creatinine levels are related to his heavy lifting and not the creatine supplementation. Remember that a by-product of heavy lifting is creatinine which is created when creatine is broken down in the muscle to produce energy, whats left is creatinine which is passed through the kidneys and excreted. 

In my opinion creatine supplementation had nothing to do with the elevated lab values which were most likely related to his intense training. I would recommend another blood draw at least 2-3 days after intense training, but he may need even longer depending on the muscle damage and the intensity of his training. However, there could be more going on here as well. If his BUN/Creatinine Ratio was not high it would support the idea that his kidneys are fine and the elevated blood creatinine levels are due to the heavy lifting.
